Nursa - Designing a Safer, More Supportive Error Reporting System for Nurses

I led the design of the Admin Dashboard, OSHA reporting workflows, and collaborated on the design of the AI chatbot assistant.

[The Problem Space]

In many hospitals, medical errors go unreported — not because nurses don’t care, but because the systems asking them to speak up are intimidating, clunky, or silent in return.

Nursa was designed to change that.

[The Solution]

Designing for Ease, Safety, and Support in Every Step

Nursa was designed around a simple premise: reporting should feel like a step toward improvement — not punishment. Each core feature was crafted to reduce cognitive load, boost confidence, and bring nurses back into the loop.

This intuitive, nurse-first error reporting platform was built to reduce the friction, fear, and frustration that surround incident reporting. From improved feedback loops to accessible language and interface design, Nursa helps turn what used to be a burden into something safer, easier, and more actionable.

“This project was created by a small UX team — myself and one other designer — with guidance from my manager during my UX co-op at Centene Corporation. While much of my official work there is under NDA, Nursa was developed to explore those challenges further. It was presented during my final co-op showcase and was well-received by stakeholders for its clarity, relevance, and potential impact.”

01 Nursa Dashboard – A Clear Overview Without the Clutter

A personalized dashboard provides nurses and admins with a visual summary of reports from the past month, including:

  • Charts showing frequency, severity, and resolution time

  • Reminders and calendar widgets to check status updates or revisit drafts

  • Activity logs for transparency and shared awareness

    → Solves: Lack of feedback and poor visibility after submitting reports

02 Streamlined Reporting with Auto-Fill Integration

To reduce friction, the system pulls basic nurse and shift details directly from HR systems. All pre-filled info is editable and accompanied by a confirmation prompt: “Is this correct?”

→ Solves: Tedious forms and risk of user errors when under time pressure

03 Built-in AI Chatbot for Guidance and Clarity

A context-aware assistant helps users navigate the platform. It:

  • Explains form sections and widgets

  • Offers OSHA-compliant advice

  • Walks users through what’s missing

  • Answers questions at any time — no need to abandon the form

→ Solves: Fear of “doing it wrong” and confusion about complex reporting steps

04 Tone, Language & Privacy Designed for Psychological Safety

Nursa uses empathetic language, avoids blame-heavy labels, and allows private reporting with draft-saving and delayed submission options. Nurses are reminded that “every report helps protect people” — not that they made a mistake.

→ Solves: Underreporting caused by embarrassment, fear of judgment, and emotional stress
